Blooming Health partnered with Project FIND and Breaking Ground to improve communication, engagement, and data collection for older adults in affordable and supportive housing across New York and Connecticut.

By working with Blooming Health, Project FIND and Breaking Ground were able to scale communication, improved tenant engagement, and streamlined outreach efforts, allowing them to better serve the older adults participating the housing program.

With messages, reminders, and surveys accessible in over 80 languages, these organizations gained valuable insights into food security, transportation access, and wellness needs—saving staff time while increasing service participation.
