In 2024, Blooming Health joined the Clinton Global Initiative (CGI) with a major goal: scale our services to 16 states and reach 2 million underserved individuals with social care by 2025. We're proud to announce that we've not only met this goal, we've shattered it, reaching over 5.5 million individuals and expanding our presence to 25 states.

We work with more than 1,200 community programs, including community care hubs, social care networks, health systems and plans, government agencies, and community based organizations. These partnerships are crucial because they connect community members to vital care services. People are more likely to engage when the outreach is coming from organizations they already know and rely on.

Our partnerships with community care hubs and social care networks have been particularly transformative. These hubs leverage infrastructure funding from Medicaid waivers and contract directly with healthcare providers to deliver social services. Our platform gives these networks the capability to reach people at scale, closing the loop on service delivery, and creates seamless experiences for members while maximizing staff efficiency.

We help community health workers increase their capacity by 10-100X when serving community members with social care navigation, while simultaneously closing critical care gaps. For example, we ensure an older adult can attend their healthcare appointments by providing safe and accessible transportation.

Our partners report being able to engage 99%+ of their members through multiple channels (text, voice calls, and emails) in over 80 languages. This comprehensive approach delivers exceptional results such as screener response rates of 35%-90%, which is 2-3X higher than traditional outreach methods.
