Last year, Blooming Health and the Clinton Global Initiative made a bold commitment: to connect 2 million older adults and underserved community members with social service providers in  2025. This initiative aims to address health-related social needs and ensure equitable access to care for vulnerable populations.

We’re proud to share that we’ve already reached 75% of that goal, demonstrating the urgent need and impact of this work.

Increasing Program Engagement with Blooming Health

Organizations using Blooming Health’s platform have experienced significant improvements in program engagement. Partners have self-reported a 300% increase in engagement with social care services, meaning more individuals are connecting with the resources they need.

This same partner reported a 500% rise in Supplemental Nutrition Assistance Program (SNAP) benefits enrollment has enabled families to access essential food assistance more efficiently.

Blooming Health’s platform increases program engagement by eliminating barriers that prevent underserved populations from connecting with vital services. Our automated communications can send multilingual communications making it accessible to individuals with limited English proficiency.

It also reaches people through their preferred communication channels, reducing missed messages and improving response rates.

Automated reminders and follow-ups keep members engaged without relying on manual efforts from staff. All this to ensure more individuals stay informed and connected, ultimately driving higher engagement in social and health programs.

Enhancing Service Coordination with the 1115 Waiver

A New York-based social care network is leveraging Blooming Health to implement the state's 1115 Medicaid waiver, a $3.1 billion initiative aimed at improving access to social health services such as transportation, nutrition, and housing. 

Previously, staff relied on cold calls to inform individuals about their eligibility for social programs—an approach that often led to skepticism and low response rates. Blooming Health’s automated and trusted outreach has helped build trust with marginalized communities, significantly increasing program participation and member satisfaction feedback. 

For high-risk populations, including individuals experiencing homelessness or those distrustful of the system, this technology offers a non-intrusive and effective way to connect them with essential services.
